The process for ordering the DecisionDx-GBM test requires the physician to perform two steps.

STEP 1:
Download the Requisition and Instructions form located to the right.
STEP 2:
Complete the requisition form, sign, and fax with a copy of the patient's insurance information to 1-866-712-5207.

Castle to initiate benefits investigation, if indicated (not appropriate for Medicare patients);
Castle to obtain and process patient specimen.
STEP 4:
Patient results will be sent to the physician(s) indicated on the Requisition Form.
