The DecisionDx-UM assay can only be ordered by the treating physician.

The DecisionDx-UM assay can be performed on uveal melanoma specimen sample obtained from either a fine needle biopsy (FNAB specimen) performed prior to brachytherapy or on a globe immediately post-enucleation. FNAB specimens require use of a specimen collection kit (supplied by Castle) to minimize the likelihood of technical failure, as such it is essential that physicians interested in the DecisionDx-UM assay have ordered and received this kit prior to sample collection.

The DecisionDx-UM assay can also be performed on tumor specimen obtained from an adequately preserved formalin-fixed, paraffin-embedded globe (FFPE specimen). Castle will obtain unstained slides from the appropriate lab.

The nature of collecting and shipping frozen samples requires close coordination between the ordering physician and our laboratory. Therefore, if you are a new customer, we request that you call our customer service line (866-788-9007) to review the ordering and shipping process prior to the placement of an order. This should eliminate errors or omissions in the collection and transport of the specimens.

 Complete the requisition form, sign, and fax with a copy of the patient's insurance information to 1-866-712-5207.
